Data-collection software for Apple's Newton PDA (personal digital assistant), OmniForm ($1500; additional Newton runtime applications, $150 each) lets you create intelligent forms in Windows or on a Mac for downloading to the Newton. After you load the form on the PDA by modem or serial cable, you complete the fields as they appear on the Newton's screen; you record answers by writing or tapping on the screen.
